Service Description:
|
- Diagnosis and care of inpatient and outpatient patients with palliative care needs
- In-hospital teams at Kingston General Hospital and Providence Care Hospital
- In-hospice care at AB Smith Homestead House (residential hospice)
- Out-patient cancer clinic at Cancer Centre of Southeastern Ontario and a satellite clinic in Belleville Cancer Centre
- Out-patient care at Lung Diagnostic and Assessment Program (LDAP) at Hotel Dieu Hospital
- Out-patient care at Providence Care Neuromuscular Clinic
- Community palliative care consult team
- Other non-malignant outpatient clinics
