Service Description: * advocates on behalf of individuals to help them access and maintain community supports and services* provides information about resources and makes referrals to agreed-upon service agencies* provides individuals with information on their rights and encourages anyone living with disabilities to acquire self-advocacy skills* advocates on behalf of individuals in order to access and maintain community support services* can provide case coordination, case management, and consultative services* liaises with other service providers to determine and address service issues and gaps

Application: Referrals go through Extend-a-family who does the assessment * clients are referred through Developmental Services Ontario, and need to pre-register to qualify for services
Eligibility / Target Population: Adults 18 years and older who have a developmental disability and are living independently in Kingston/South Frontenac County
